Resolved Question: Question about a health insurance application I'm applying for?

To answer your question with a knowledgeable answer, I would have to see the application.

The app might ask questions like:

When did you see a doctor last? What were the results of your visit?

Or, Have you seen a doctor for anything else in the past 12 months?

My question is this. Where did you get the application? Why isn't an agent filling it out?

A licensed agent should fill out he application to make sure the questions are understood, and the correct answer is written down. Sometimes an agent will understand a question a different way than the applicant, because of training, unless the insurance company mailed you the application to personally fill out.

EDIT:

Google Assurant Health, or Golden Rule Insurance, then do a search for a local agent.
