Resolved Question: Can I add my girlfriend to my health insurance if we live together?

There are several levels to this "domestic partner" benefit. First, state law has to allow it. Then, the insurance carrier has to allow it. Lastly, your company has to allow it. It has nothing to do with "common law", but all will require that you meet certain criteria. It also has nothing to do with same-sex marriage or civil unions.

The easiest place to get an answer to this question is the Human Resources department of your company. They will know if the state, the insurance company, and they allow it. You may have to wait until open enrollment in order to add her to your insurance, if allowed, unless some qualifying event happened, like she lost prior coverage. You will have to provide proof of that prior coverage.

How to find the best, low cost health insurance plan? I can't tell you how important it is to do your homework, and in this article I'll try to help you with some of that homework. You might think health insurance is all the same, but the devil is in the details and understanding the details may save you hundreds of dollars.

When you buy health insurance, just as with any insurance, you are paying the company a monthly fee (insurance premium) to manage the risk of your need for health care coverage. The more risk the company assumes, the greater the premium. However, you as a consumer, must understand what you are paying for, and you also have to be your own watchdog to some degree and pay attention that you get what you pay for. At its basic level, health insurance is the assumption of risk on the part of the company.

Health insurance comes in many different forms. For example, there is disease insurance, accidental death and dismemberment insurance, catastrophic health coverage, COBRA insurance, and maternity coverage to name just a few examples. All of these, by the way, are kinds of health insurance. When you shop for health insurance you are generally presented with a variety of plans that offer different benefits and and different levels of coverage. Insurance plans are the way the insurance is packaged. The plan is the "bottle" holding the wine, as it were.

Some of the more common kinds of health insurance plans are the Health Maintenance Organization, or HMO; the Preferred Provider Organization, or PPO; and the Private Fee for Service Plan, or PFFS. HMO plans are generally less expensive, but they required that you use only the doctors, hospitals, and other health care providers who have a contract with the insurance company to provide service. You are usually assigned a primary care doctor and must get a referral to see a specialist. The plan ultimately determines whether or not you can see a specialist and what services and how many of each service you may receive. If you are in relatively good health and have relatively few medical needs, an HMO might work out for you. If you have a more complicated medial history, you would want to thoroughly investigate the insurance company and specific HMO to be sure they have a reliable record of meeting patients coverage needs. Kaiser Permanente would be an example of a well reputed HMO, and Kaiser Health Care generally has a good reputation throughout the United States where it is offered.

The PPO is more flexible than the HMO plan. PPOs may also be a bit more expensive than HMOs, and that is because PPOs are generally thought to give a higher level of coverage. In PPOs, you are not required to have a primary care doctor, and you generally do not need a referral to see a specialist. PPOs have their own network of contracted medical personel but allow you to go out of the network to see doctors of your own choosing. Now, that costs you more. That means the company provides a lower level of coverage (payment) and make up the balance. Before signing up for a PPO, it is important that check with your providers to determine whether they are in the network or will otherwise file claims and accept payment from that particular insurance company. Major insurance companies such as Aetna and Blue Cross Blue shield, Wellmark Blue Cross, and Humana offer PPO plans as well as HMO variants.

A PFFS plan is still a kind of managed care, but in the private fee for service plan, you go to any doctor or hospital you choose as long as they submit claims to to insurance company and accept payment. In a fee for service plan, your health care providers would bill the insurance company a specific fee for each service provided. What the insurer pays is based on a fee schedule.

When you buy a health insurance plan, the actual cost of the plan is not only the price of the premiums, deductibles, co-pays and co-insurances. When you figure the real cost, you must also take into consideration the reliability of the company in living up to their promise of coverage for the kinds of services that are important for you and your family. Thus, a "cheap" plan could end up costing you more if the company doesn't cover the costs specified in the policy. Humana, for example, may offer you less expensive plans, but if you have to argue with them over meeting the basic agreements in the policy, then the coverage would be useless and the cost to you far greater than you had imagined.

You can find cheap quotes for health insurance, but the key to a low cost plan is in coming as close as you can to paying only for a relatively few number of key services. In other words, if possible, you strip the policy of every service you can possibly do away with and assume as high a deductible as possible. Resolved Question: Question about a health insurance application I'm applying for?

To answer your question with a knowledgeable answer, I would have to see the application.

The app might ask questions like:

When did you see a doctor last? What were the results of your visit?

Or, Have you seen a doctor for anything else in the past 12 months?

My question is this. Where did you get the application? Why isn't an agent filling it out?

A licensed agent should fill out he application to make sure the questions are understood, and the correct answer is written down. Sometimes an agent will understand a question a different way than the applicant, because of training, unless the insurance company mailed you the application to personally fill out.

California Health Insurance Executives Testify Before Congress in March


Congress is scrutinizing Health Insurance for California by challenging the reasons that prevent many people from obtaining health care coverage. When the largest health insurance company in the U.S. (WellPoint, Inc.) proposed raising monthly premiums by up to 39 percent in California, Congress summoned WellPoint chief executives. WellPoint postponed its premium increases until May 1st 2010, and California's insurance commissioner will decide whether to allow the increases.

In short order, Congress expanded investigation of Health Insurance in California to look at coverage being refused and medical claims being denied as "preexisting conditions." Chief executives from three more of the country's largest for-profit health insurers (Aetna, Humana, and UnitedHealth Group) were summoned to appear before the House Committee on Energy and Commerce and its investigations subcommittee on March 23rd.

Congress asked the companies to provide internal documents and e-mails about guidelines, practices, and underwriting policies for the last five years. These companies have been asked to show average premiums and increases, as well as maternity coverage for individuals by March 12th. These four companies sell health insurance policies for individuals to approximately 17 million Americans without health coverage from work.

Need for Maternity Coverage in California Health Insurance Plans

Looking at maternity coverage is important because woman ages 18 to 29 are the most likely group in the U.S. to lack health insurance. With just 59 percent of these women having health insurance through an employer, these young women comprise the group that is least likely to have health insurance from work. One-fourth of these women have no health insurance at all, yet 3.5 million pregnancies occur among the 21 million women who are ages 19 to 29 each year.

As in third-world countries, lack of health care hits babies the hardest. Uninsured pregnant women are 60 percent more apt to delay prenatal care, three times more likely to suffer adverse outcomes, and their babies are 30 percent more likely to die.

Premiums and benefits vary greatly between insurance companies. Even insurance companies that are raising premiums, may have alternative plans with higher deductibles and lower premiums.

Look for Websites that compare plans and offer quotes from a range of companies to get a feel for benefits and prices. Independent agents (those who work with many different insurance companies) may know which insurers have more lenient underwriting for people with back problems, for instance. Independent agents can also appeal if you are denied coverage, or are assessed a higher rate because you have pre-existing conditions.

Compare California Health Insurance Plans to Get the Most for Your Money

When comparing policies look at the annual deductible, the percentage you pay for a medical service after a deductible is met (coinsurance), and the annual out-of-pocket maximum. That maximum is how much you have to spend on co-pays, coinsurance, and other costs before the insurance company covers any expenses for the year.

Check whether doctor visits are covered, and whether hospital stays have limited coverage. Look at which doctors and hospitals are in-network, too. Some individual plans exclude maternity coverage, but separate policies deal specifically with maternity and prenatal care.

If you aren't taking prescription drugs, you can save by eliminating that coverage. If you are using prescription medications, research the cost of buying through discount drug programs at King Soopers, Wal-Mart, or other large chains.

Be careful about letting coverage lapse while you're looking for a better policy. Events during a coverage lapse might make you ineligible for coverage. For example, insurance companies may not cover birth or prenatal care if there has been a gap in coverage.

Maternal and child services are also available for women with low incomes who are pregnant, and have children under the age of 22. A program called Women, Infants and Children (WIC) provides education about nutrition and child care for woman with low incomes, woman who are breastfeeding, and postpartum women. Even if you do not qualify for the above programs, you may still have access to care through free clinics, prescription drug assistance plans, or temporary state insurance. Your best bet may be to educate yourself about how health care insurance works, and find an independent agent that will take the time to listen to your needs and help you navigate the insurance jungle.

Five Companies That Provide Health Insurance in Utah


We here in Utah have some of the best options for health insurance companies in the country. Those options are what we are going to discuss here and how to navigate through everything. One thing to remember though is what may be a great provider and plan for your friend may not be a great option for you.

There are five main providers here; SelectHealth, Humana, Regence Blue Cross Blue Shield, Altius and Assurant. Let's go into a little detail on each, shall we?

The largest provider in Utah is SelectHealth. Most people know them as Intermountain Healthcare,  IHC or by their plan names of SelectValue, SelectMed and SelectCare. They have over 3700 participating doctors and the most hospitals in the state. They are also the administrators of HIPUtah. They offer a number of different plans that can include maternity coverage, dental and vision.

Another carrier here is Humana. Humana offers a broad range of plan options and also have a very large doctor and hospital network. Since Humana is a national insurance provider, their network is available nationwide. What that allows for is if you are traveling out of state, you will be able to find doctors that are in network there. That will help you keep your medical costs down.

Regence Blue Cross Blue Shield is a regional provider that offers a network available in Utah, Idaho and Washington. They offer many different plan types, including HSA, PPO and traditional plans.

Other similar carriers are Assurant and Altius. They all offer a variety of plans to suit your needs, but they also offer what are called traditional plans as well. These were the types of plans that were available prior to PPO and HMO type plans. There is no network of doctors you have to go to. You are able to pick whatever provider you wish and be treated in any hospital. These plans are more expensive, but they allow the freedom you may need.

One thing to remember about companies here in Utah is the networks that they use. IHC hospitals and facilities use only SelectHealth as an in network carrier. What that means is that the largest health care provider in the state only uses one health insurance company (their own) as able to use their facilites at the lowest rates. Now many of the doctors that are IHC doctors are not necessarily only in network for SelectHealth and you need to double check to make sure what networks they are in. So if you find that your IHC doctor is in a number of networks, and you find great coverage with a lower premium with another company, you will have to travel to an in network facility to receive the lowest cost of care. The funny thing is that all non-IHC facilities will accept SelectHealth in network.

Each of these carriers has strong points that fit for certain needs, health conditions and pricing for each family. As we have said before, there is no one size fits all. To help you sift through all of the different providers and their numerous plans, I recommend speaking to someone who knows what would work for you, like a independent insurance agent that is based in Utah. You can either love a job or hate a job at any insurance company. It depends upon many things. Check out their reputation as a company. Start by finding out their A. M. Best rating. If it is any "A" rated company, then it is financially strong. A "B+" rated company is also reasonably strong. The lower the rating gets, the less financially strong the company will be. If it is not strong, then it might not be the most stable company to work for. However, financial stability is not the only factor to consider. You will also want to know about the benefits package they have for the employees. Most insurance companies have good benefit packages.

Then consider this, with any company, how well you like it there is largely dependent upon how you like your job and how you get along with your co-workers. If you are in a job you don't like, you won't be happy. If you have co-workers your do not like, you won't be happy. Humana is a large company with offices in many states. No one person can tell you whether your will be happy there or not. They don't know you and, unless they are in the same department in the same office as you would be working in, they can't tell you what your co-workers are like.

I work for a property and casualty insurer. What I like is that we have a very good benefits package. We are financially strong (current A. M. Best rating is "A-", working toward "A" rating.) The management is concerned about the employees. The supervisors either deal equitably with their subordinates or find themselves out of a job. Most of my co-workers are pleasant (well, there have to be a few that aren't as nice sometimes). The company does its best to maintain a pleasant working environment. Well, I can think of a number of other things to say here. However, they are things that any good employer should be doing.

What didn't I like about where I work? I didn't like one supervisor. She was so difficult that she got her supervisor demoted back to his old position because he couldn't handle her. Her next supervisor was given the opportunity to resign because he couldn't handle her. Whenever she didn't get along with someone, it was always their fault because she was never wrong. When our department was combined with another department that did the same thing as we did except with different sized accounts, they put her in a different place in the company. When they decided to eliminate that part of the company, every one else in the department was offered a different position in the company but her. No one would interview her for another position because everyone figured out that she was the one causing all of the problems.
